Referral Process
Ophthalmology Associates, S.C. is proud to partner with Optometrists and Primary Care Doctors throughout all of Southeastern Wisconsin. Our goal is to provide an easy, seamless referral process for you and your patients. We are committed to working as your trusted partner in caring for your patient and will keep you informed of our diagnosis and treatment plans every step of the way.
For general referrals, please fill out the above referral form and fax to 414-281-0959.
For urgent referrals, please call our office directly at 414-281-0424 for assistance with scheduling.
For corneal referrals to Dr. Hart Moss, please return the above referral form, and mail color copies of all topographies and diagnostic imaging to our Medical Records Department, at the address listed below. Please include all pertinent medical records, including past refractions and keratometry.
