You will need an itemized receipt that shows your exam date and your contact lens purchase. Don’t have one? Give our office a call and we can send it to you!

- Bausch + Lomb rebates (i.e. Biotrue, PureVision, Ultra)
- CooperVision rebates (i.e. Biofinity)
